Hi Bobcats,
Prior to the season starting, you must do the following:
Get a physical done by a physician
- Download the Preparticaption Physical and go to a doctor to complete.
- Preparticipation Physical- Page 1
- Preparticipation Physical- Page 2
Then complete the online clearance with the link below. This must be done every year!
